QACPS' first priority is student safety and security. Our ability to provide safety and security is dependent on the completion of the Back to School Forms.
We are pleased to announce that all of the Back to School Forms for the upcoming school year will be done electronically! This process replaces the paper forms sent home at the beginning of each school year. We depend on having the information in these forms to ensure your student’s safety and security in school throughout the year.
The 2017-18 Back To School Forms for Queen Anne’s County Public Schools are now available online. Please complete the online back to school forms for each child you have enrolled in Queen Anne's County Public Schools.
- Middle School and High School students and parents will not have access to their schedules via Parent Portal until the Back to School forms are completed online.
- Elementary School parents will not have access to their student’s attendance or teacher(s) email links via Parent Portal until the Back to School forms are completed online.
Parent Portal will be turned back on the week of August 14, 2017, for students and parents who have completed the forms online. It is important to complete these online forms because this allows your child's school to know important medical information, where the bus is to pick your child up, emergency contact information and allows your child to use electronic devices in school.

How do I get started?
Visit our district website at http://qacps.schoolwires.
net/ and click the 2017-2018 Back to School Forms link (or see above link at top of this page). Then, enter your student’s snapcode
What’s a snapcode?
The snapcode is like a key to your child’s 2017-18 Back To School Forms for the upcoming school year. You should receive a unique snapcode for each child each year.
Where do I get my student's snapcode?
An email with your student's snapcode information has been sent to the email address you registered last year. You may also call your student's school's front office to be assisted with getting your snapcode. All students received their snapcode are on their most recent Progress Report (elementary) or Report Card (middle and high), as well.
Should I create an account?
- If you’ve never completed an online 2017-18 Back to School Forms online, you should create an account. This allows you to securely save your work and come back at a later time if necessary. You can use your email address or cell phone number.
- If you already have an account, you can sign in and complete the form. (You should use the same account to complete forms for multiple children.)
Do I have to answer all the questions?
Questions marked with a red asterisk (*) are required.
What if I make a mistake?
If you would like to make a change, click on the underlined field or click “< Prev” to return to a previous page.
I’ve completed the form, now what?
When you have finished entering your information, click “Submit.” This will send all of the information you’ve entered to the school. If you cannot click on this button, you will need to make sure that you have answered all required questions.
What if I have more than one student in the district? Do I need to do this for each child?
Yes, because you’ll need to provide information that is specific for each child. We recommend that you submit one 2017-18 Back To School Forms and then start another – this will allow you to “snap over” shared family information, which will save you time.
