    THE FOLLOWING INFORMATION IS REQUIRED AT THE TIME OF REGISTRATION:

    1)  Student's Birth Certificate

      2) Student's Social Security Card/number

    3)  Student's Immunization records

      4) Proof of Residency (only one of the following is required;)

    contract of sale  

         electric/power bill

    gas bill

    homeowners/renters insurance statement

    lease/rental agreement

    mortgage statement

                   Queen Anne's County property tax bill

    water bill

    ******* Not accepted; bank statement, cable bill, cell phone bill

    Please note: report cards and test scores are helpful, but not required
