All consent forms must be turned in by Friday 9/22/23
Please turn in all consent forms and the notice of privacy act, all forms must be fully completed and signed by a parent/guardian for a student to receive the Flumist.
Consent forms can be found on September 8th’s Crab Courier link sent to parents/guardians via email from Mrs. Cornish. Consent will also be sent out via school messenger on Thursday, September 14th, 2023.
